Clinica Medica Familiar is seeking a dedicated and compassionate Primary Care Provider (NP or PA) to join our established, high-volume family practice serving the Montebello community. Our clinic provides comprehensive care to a diverse patient population, including a large Medi-Cal membership, where providers have the opportunity to make a meaningful difference in community health every day.

This is an excellent opportunity for a provider who enjoys true primary care medicine, continuity with patients, and working within a collaborative team environment.

Position Overview

The Primary Care Provider will deliver comprehensive medical care to patients across the lifespan, focusing on preventive care, chronic disease management, and patient education.

Responsibilities

• Provide comprehensive primary care services including evaluation, diagnosis, and treatment of acute and chronic medical conditions
• Perform routine physical examinations and preventive health screenings
• Manage chronic conditions such as diabetes, hypertension, and cardiovascular disease
• Order and interpret diagnostic tests and laboratory results
• Prescribe medications and treatment plans within scope of practice
• Provide patient education focused on prevention and lifestyle improvement
• Coordinate care with specialists and other healthcare professionals
• Maintain accurate and timely medical documentation
• Support quality care initiatives and population health measures

Qualifications

• Current California license as Nurse Practitioner or Physician Assistant
• DEA registration
• Board certification required
• Experience in Family Practice
• Bilingual Spanish required
• Strong commitment to serving diverse and underserved communities

Medical service is available at a nominal fee per cash visit. The clinic accepts many forms of medical insurance including Medi-Cal, MediCare, Healthy Families, and most private insurance. FREE programs are available to those that qualify.
