Position Summary

We are seeking a Licensed Practical Nurse (LPN) to join our Home Health team. In this role, you will provide high-quality, patient-centered nursing care in the home setting, working alongside an interdisciplinary team to support patients on their journey to wellness and independence.

This position is exclusively home health—no facility work—allowing for meaningful one-on-one patient relationships and autonomy in your daily work.

Key Responsibilities

• Deliver skilled nursing care to patients in their homes under the supervision of an RN
• Administer medications, treatments, and wound care as prescribed
• Monitor patient conditions and report changes promptly to the RN and care team
• Educate patients and caregivers on care plans, medications, and disease management
• Accurately document all patient interactions in compliance with regulatory standards
• Coordinate care with physicians, therapists, and other healthcare professionals
• Support patients in achieving optimal health outcomes and independence

Qualifications

• Current, unencumbered LPN license in the state of Oregon
• Minimum 1 year of nursing experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently in a home-based setting
• Valid driver’s license, reliable transportation, and current auto insurance
